7 Signs You Might Need a Root Canal (And When to See a Dentist)

Tooth pain has a way of announcing itself at the worst possible moment — and it’s tempting to wait it out, hoping it settles on its own. Sometimes it does. Sometimes it’s your tooth telling you the nerve inside is in trouble, and waiting only makes the eventual treatment more involved. Here’s how to tell the difference.

1. Pain That Lingers After Hot or Cold

A quick zing from an ice cream or hot tea that fades in a few seconds is usually just sensitivity. Pain that lingers for 30 seconds or more after the hot or cold is gone is a different story — it often means the nerve inside the tooth is inflamed or infected.

2. Pain When You Bite Down

Sharp pain specifically when chewing or biting can point to a crack reaching the pulp, or infection at the root tip putting pressure on the tooth from below.

3. A Dull, Constant Ache

Unlike sensitivity that comes and goes, an ache that’s simply always there — even mild — often means inflammation has set in and isn’t resolving on its own.

4. Swelling Near the Tooth

A small bump on the gum (sometimes called a “gum boil”) near a specific tooth is usually a sign of infection draining from the root. It may not even hurt much, which makes it easy to ignore — don’t.

5. Tooth Discolouration

A single tooth turning noticeably darker or greyer than its neighbours can indicate the internal pulp tissue has died, even without significant pain.

6. Pain That Wakes You Up

Dental pain that’s bad enough to disturb sleep is rarely something to “wait and see” about — it usually means active inflammation or infection that needs attention soon.

7. A Cracked or Visibly Damaged Tooth

If a tooth has fractured deeply enough that you can see (or feel with your tongue) into the inner structure, the nerve is very likely exposed or at serious risk.

What Happens If You Wait?

Untreated infection doesn’t stay contained — it can spread into the surrounding jawbone, cause a painful abscess, and in some cases mean the tooth can no longer be saved at all, leaving extraction as the only option.
